2023-2024 Texas Residency Affirmation. If you are a Texas resident, this core question form is required to verify your state residency. Pending availability of state funds, certain financial aid programs are available to LeTourneau University students who are bona fide residents of Texas. The form must be completed in its entirety.

2023-2024 Special Circumstance Request. This form is for reporting financial changes that occurred that will affect the 2023-2024 academic year. Conditions that do not warrant an appeal would include payment of routine living expenses (house payment, car, credit card, etc.) debt repayment and other expenses that reflect lifestyle choices. Special circumstances may include something like excessive medical expenses, younger children in private schools, loss of income, divorce, or death in the family. Once the financial aid office has received all required documentation, students will be notified within 30 days with the results of their request. .

2023-2024 Verification Worksheet. This form is required from all students who are selected for the verification process. If the tax documentation is required in the verification email, then also submit an official tax transcript.

2023-2024 Federal TEACH Grant application form. The Teacher Education Assistance for College and Higher Education (TEACH Grant) is a federal grant program for undergraduate and graduate students who plan to teach in a public or private elementary or secondary school that serves students from low-income families. Once you complete the application it will be forwarded to your Academic Advisor through Dynamic Forms for initial approval. The initial application will go through several approval steps to ensure that you meet the qualifying criteria to be eligible for the Federal TEACH Grant. This can take several days so we thank you ahead of time for your patience. Once these steps have been completed, financial aid will send you an email about the results for your initial application.  If you have any questions, please feel free to contact your Academic Advisor or the financial aid office. 

2023-2024 Church Matching Grant handbook and form. The Church Match Grant Program is a student aid program at LeTourneau University in Longview, Texas. By participating in the program, your church enters a “partnership” with LeTourneau to provide financial assistance to your students who attend LeTourneau University. LeTourneau will match qualifying church scholarships with an equal Church Match Grant, to a maximum match of $1,000 each year ($500 each semester) for each full-time student.

2023-2024 ID and ED Purpose Form Notary. This form is required by the Department of Education. The Financial Aid Office is unable to complete your verification process without this documentation.  In order to continue this process and to be able to offer financial aid assistance, we will need the following additional information: print and complete the notarized copy of the ID and ED Purpose Form Notary. This form must be completed by a certified notary public. Please be sure to bring current legal proof of identity with you (i.e. driver’s license, passport). The notarized form and a copy of the identifying document used must be mailed to the Financial Aid Office in Longview.  Our address is on the form.

IRS Form 4506-T Request for Transcript of Tax Return. This form is to be completed by students, parents and/or spouses that did not file a 2021 tax return. Instructions for completing and submitting the form are included in the download.

2023-2024 Document Upload Form. This form may be used to send additional documents that are needed in a secure manner. (social security card, driver’s license, birth certificate. etc.)
